CPT code 99211 is an office or other outpatient evaluation and management code for an established patient, describing a minimal encounter that may not require the presence of a physician or other qualified health care professional. It applies only where a genuine E/M service is performed, is medically necessary, is documented, and is permitted under the payer’s supervision rules.
CPT code 99211 attracts more incorrect claims than almost any other office E/M code, and the reason sits in what was removed from it rather than what it contains. It carries no time threshold and no medical decision making requirement, which invites the reading that it has no requirements at all.
The requirement did not disappear. It moved to a question that is harder to answer from a schedule: did an evaluation and management service actually take place? A patient who came in, had something done and left has generated an encounter, and an encounter is not automatically a billable visit. This guide covers what the code describes and the test that decides whether it applies. It then sets out who may perform the service and under what supervision, what documentation has to show, and where these claims commonly fail.
What Does the CPT Code 99211 Describe?
The descriptor covers an office or other outpatient visit for the evaluation and management of an established patient. It describes a service that may not require the presence of a physician or other qualified health care professional.
Established Patient Only
A patient is generally established where they have received a professional service from the billing provider within the previous three years. The same applies where that service came from another physician of the same specialty and subspecialty in the same group practice.
A new patient receiving the same minimal service does not fall to 99211, since 99201 no longer exists and there is no new-patient equivalent at this level.
No Time Or Decision Making Criteria
The other office E/M codes are selected on total time or medical decision making. 99211 is not, and the reference to a typical five minutes that many practices still work from was removed when the office E/M codes were revised.
Anyone still choosing the code because an encounter was short is applying a rule that no longer exists. Anyone avoiding it because an encounter ran long is doing the same thing in reverse.
May Not Require A Physician’s Presence
The descriptor says the service may not require the physician’s presence, which is a statement about the nature of the service rather than a grant of independent billing. Supervision rules still govern, and those come from the payer.
The Test Is Whether An E/M Service Happened
Something has to be evaluated, and something has to be managed. Both parts matter, and most rejected CPT code 99211 claims fail one of them.
| Encounter | Usually supports 99211 | Why |
|---|---|---|
| Blood pressure check with assessment and a documented plan response | Yes | Evaluated against a plan, action taken or confirmed |
| Wound check with assessment of healing and instruction | Yes | Clinical assessment and management occurred |
| Medication review with adherence assessment and counselling | Yes | Evaluation and management of an ongoing issue |
| Blood draw only | No | A separately reportable technical service |
| Injection administration only | No | The administration code covers the work |
| Patient collecting a prescription or test result with no assessment | No | No evaluation, no management |
| Weight taken and recorded with no clinical response | No | Data collection rather than an E/M service |
Info: Ask what changed as a result of the encounter. If a note records a value and nothing else, the visit produced data rather than management, and the difference is what an auditor looks for.
Who May Perform The Service, And Under What Supervision
Clinical staff commonly perform the encounters behind a CPT code 99211 claim, and that is permitted. What governs is the supervision arrangement under which the service is billed.
Under Medicare’s incident-to rules, a service performed by auxiliary personnel and billed under a physician carries three conditions. The physician must have performed the initial service establishing the plan of care, the staff member’s work must follow that plan, and direct supervision must be in place at the time.
Direct supervision has specific meaning, and it has also been subject to temporary flexibilities in recent years. Confirm the current definition and any active flexibility before relying on it, since this is the element most likely to have changed since a practice last reviewed its policy.
State scope of practice rules and commercial payer policies apply on top of all of that, and they differ.
Note: Incident-to is a Medicare billing concept, not a universal one. Commercial payers may apply their own supervision and staffing requirements, and some do not recognise incident-to billing at all.
What The Documentation Has To Show
A note supporting CPT code 99211 needs to demonstrate that an E/M service occurred, which a vital sign and a signature do not.
- The reason for the encounter, linked to an existing problem or plan
- What was assessed, including relevant findings
- What was done in response, whether that is instruction, a plan change or confirmation to continue
- Who performed the service, identified by name and role
- The supervising physician, where the service is billed incident-to
- The date of service and a legible signature
The middle two items are where audits concentrate. A note reading “blood pressure checked, 138/84” records a measurement. A note recording that the reading was assessed against the treatment plan, that the patient was counselled on adherence and that the current dose continues describes a service.
Where 99211 Claims Go Wrong
Four failures account for most CPT code 99211 denials, and three of them are avoidable at the point of coding.
- Reported alongside a bundled procedure. Edits bundle 99211 into many services, and reporting it with an injection administration or a venipuncture on the same day is a frequent denial. Where a separately identifiable E/M service genuinely occurred, the documentation has to support it independently.
- Used where a higher level is supported. A staff-performed encounter is not the same as a minimal one. Where a physician or qualified professional evaluated the patient and the documentation supports a higher code, reporting 99211 undervalues the work.
- Billed without the supervision requirement met. The physician being somewhere in the building is not the same as the applicable supervision standard being satisfied.
- Applied as a default for nurse visits. The phrase nurse visit describes who provided care rather than what was performed, and it is not a coding category.
Final Thoughts
CPT code 99211 is not a low-requirement code, it is a code whose requirements sit somewhere people do not look. There is no time threshold and no decision making level. The question that decides it is whether an evaluation and management service actually happened, rather than how long the patient was in the room.
Apply the test to the note rather than the schedule. Something must have been assessed, and something must have been done in response. That is why a recorded vital sign alone does not support the code while a documented assessment against a plan usually does. Confirm the established patient relationship, confirm the supervision arrangement against the specific payer, and check the edits before reporting it alongside another service on the same day.

Frequently Asked Questions
Can A Nurse Bill CPT Code 99211?
Clinical staff can perform the service, but the claim is billed under a supervising physician or qualified health care professional according to the payer’s rules. Nurse visit describes who provided the care rather than what was performed, and it is not itself a basis for reporting the code.
Does 99211 Still Have A Five Minute Time Requirement?
No. The typical time reference was removed when the office and outpatient E/M codes were revised, and 99211 is not selected on time or on medical decision making. Practices still applying a five minute rule are working from a superseded version of the descriptor.
Can 99211 Be Billed With An Injection Or Blood Draw?
Usually not on the same day, since edits bundle it into many procedures and the administration or collection code covers that work. Where a separately identifiable E/M service genuinely took place, the documentation must support it independently of the procedure.
What Documentation Supports 99211?
The reason for the encounter linked to an existing problem or plan, what was assessed, and what was done in response. Also who performed the service, the supervising physician where billed incident-to, and the date with a legible signature. Assessment and response are where audits concentrate.
